Case Managers assure full coordination and linkage of services provided by your choice of over seventy (70) available agencies approved by the State of Florida and local Area Agency on Aging. Case Managers will assist with applications for food stamps and other needed services available in the community.
Case Managers involve, with client’s consent, family and caregivers in the client’s plan of care. Case Managers assist families and caregivers to resolve the emotional, financial and physical demands of caring for their elder loved ones.
Most homebound seniors in need of long-term care services prefer to get help in their home. Experienced Case Managers provide assessments to determine senior’s individual needs and coordinate and manage the following in home services:
Homemaker: light housekeeping, shopping, errands and laundry services
Personal Care: assistance with bathing, washing hair and dressing
Respite Care: reprieve for caregivers tending for an ill or impaired elder
Companionship: in-home supervised socialization
Chore: heavy house cleaning
Pest Control: rid the house of insects and pests
Emergency Alert Response: monitoring by electronic device
Counseling: mental health counseling
Consumable Medical Supplies: disposable medical supplies and food
Specialized Medical Equipment: wheelchairs, hospital beds, etc.
Skilled Nursing: medication management, dressing changes, ulcer treatments, etc.
Attendant Care: assistance with medically related needs
Caregiver Training: educate caregivers on various skills
Home Modification: adaptations to increase safety
Escort: accompaniment to medical appointments
Home Delivered Meals: hot, frozen, kosher, diabetic low sodium, Latin, pureed, breakfast meals, 2 per day. Menu choices available
Adult Day Health Care: includes nursing, therapy and transportation
DOSS provides professional home visits. Our caring Case Managers will accommodate your schedule to provide the most convenient time for you. Home visits are conducted to review existing services and arrange additional in home assistance if needed.
